What is this Form Used For?
You will Need to Use this Form When Informing NDIS that something in relation to your life or disability have changed.It might be that you have moved and you would like update your address.It might be that you now want to be Plan Managed for your NDIS and not NDIA Managed or maybe you need more or different services and supports.This form makes it easy to let NDIS know what is happening with you and what you would like to ask of them.
Please make sure you have SUPPLIED ALL the Relevent DOCUMENTS
